Partial Pulpotomy to Successfully Treat a Caries-Induced Pulpal Micro-Abscess: A Case Report

Vital pulp treatment (VPT) is a therapeutic strategy aimed at conservatively managing deep carious lesions and the exposed pulp. VPT has recently expanded through use of hydraulic-calcium-silicate cement (HCSC), cone-beam CT, dental operating microscopy (DOM), as well an improved understanding pulpal repair mechanisms. Consensus documents have concluded that non-selective removal caries not necessary in absence clinical symptoms, partial or techniques reduce likelihood exposure. Alternatively, others suggest leaving tissue may help sustain inflammation lead to failure VPT. The presence purulent discharge from considered indicative irreversible damage need for pulpectomy. This report successful symptomatic molar with exhibited after A female patient presented spontaneous pain maxillary left second molar. One year before, she had received cast-metal restoration over resin-based-composite (RBC) restoration. tooth been largely asymptomatic since; however, experiencing pain, consulted hospital clinic. Removal using DOM resulted exposure bleeding. pulpotomy HCSC performed. Six months later, symptoms completely resolved; due concerns necrosis, re-assessed. examination revealed incomplete hard-tissue barrier; moreover, there were also signs residual inflammation. As result, defected area enlarged, debrided, reapplied. After review 1 year, remains symptomless, apical radiolucency. Notably, this visually illustrates containing limited region pus can maintain vitality finding raises two questions: (1) whether infected situ avoiding visualization appropriate; (2) warrants
